What BMI Actually Measures
Body Mass Index is simply weight divided by height squared (kg/m²), sorted into categories: underweight, normal weight, overweight, and obese. It was developed in the 1830s by a Belgian mathematician for population-level statistics, not individual health diagnosis.
Why It's Still Widely Used
BMI is cheap, fast, and requires no special equipment beyond a scale and a tape measure — which is exactly why it remains a common initial screening tool in medical settings and public health research, despite its limitations. At a population level, higher BMI does correlate with increased risk of certain health conditions.
Where BMI Falls Short for Individuals
- Doesn't distinguish muscle from fat. A muscular athlete can register as "overweight" or even "obese" by BMI despite having low body fat, since BMI only sees total weight relative to height.
- Doesn't account for fat distribution. Two people with the same BMI can have very different health risk profiles depending on whether fat is stored viscerally (around organs) versus subcutaneously — visceral fat is more strongly linked to metabolic risk.
- Doesn't account for age, sex, or ethnicity variation in body composition, though some alternative thresholds exist for certain populations.
- Doesn't measure health directly — it's a proxy correlated with risk at a population level, not a diagnosis for any individual.
What to Look at Alongside BMI
- Waist circumference or waist-to-hip ratio — better indicators of visceral fat and associated metabolic risk than BMI alone.
- Body fat percentage — directly measures the proportion of fat versus lean mass, though accurate measurement typically requires specialized equipment (DEXA scan, hydrostatic weighing) beyond basic estimates.
- Blood pressure, blood sugar, and cholesterol — direct clinical markers that more accurately reflect metabolic health than any body-composition proxy.
- How you actually feel — energy levels, sleep quality, and physical capability are meaningful indicators BMI doesn't capture at all.
How to Use BMI Sensibly
Treat a BMI calculation as one data point, not a verdict — useful as a rough starting screening tool, especially at the population level, but not something to over-interpret for any single individual, particularly if you're notably muscular or have other individual factors that BMI doesn't account for. If you have health concerns, a full clinical picture from a healthcare provider will always be more informative than BMI alone.
